Grant Reporting – Final

Reporting Requirements

Under your Agreement, the Grantee is required to provide a Final Report. Final Reports are due 30 days after the end of the project (unless otherwise specified within your project agreement). The report should include before, during, and after photos; plus the following three forms:

  1. Form-GLC-GLRI-1D-Final – Invoice
  2. Form-GLC-GLRI-2A-Final – Project
  3. Form-GLC-GLRI-4-Final – Final Report Narrative

2018 Grant Recipients: You have been provided with updated forms that have been customized for your project. Do not use the forms above. If you need your forms emailed to you again, please contact Ken Gibbons (kgibbons@glc.org).

Please save your forms to your computer and, once completed, submit them below. The Invoice (Form 1D) must be a signed PDF file.The other two forms–Project and Final Report Narrative (Forms 2A and 4)–must be submitted in the original Excel and Word format. Please do not make PDFs of these two files, as they need to be editable for consolidation with other forms, and this will delay your payment.
